Director of Case Management

Job Description

There is a direct hire opportunity with a well known medical center in Titusville,FL for a Director of Case Management.

Qualifications

• Bachelor's Degree required. Masters preferred. Major: Nursing, Business Administration, or Healthcare Administration.
• Minimum of 3-5 years of experience in case management, discharge planning, utilization review or related experience. Prior experience in a management or administrative position.
• Clear and active State of Florida RN license. Certified Case Manager preferred.
• Knowledge of State, Federal, and JCAHO rules and regulations relative to case management and admission criteria.
• Knowledge of hospital based case management, transitional care program, utilization review and skill in applying that knowledge to directly impact the continuum of care to a home health agency, skilled nursing facility, hospice, inpatient rehab facility and long-term are facility.
• Knowledge of payer requirements and discharge planning regulations that support the effective development of departmental policies, procedures and standards
General Description
Under the direct supervision of the Executive Director of Information Governance, is responsible for daily supervision and coordination of case management activities and employees within the Case Management Department. Will be actively engaged in developing, executing and reporting on the applicable areas of the game plan and utilize PMC's performance improvement process to improve processes, systems and goals that are falling short of expectations, targets or best practices.
